PMMA (Presbyterian Manors of Mid-America) first offered hospice services in Farmington, Mo., to extend end of life services to residents of Farmington Presbyterian Manor and their families.

Early history

Licensed in 2005 by Medicare, Presbyterian Hospice of Mid-America staff included a medical director, administrator, clinical coordinator, one hospice aide, one nurse, one social worker, one chaplain, and one volunteer. The hospice was based in an office at Farmington Presbyterian Manor.

In late 2005, a duplex located on the Farmington Presbyterian Manor campus was converted into an office for the hospice program. The move gave Presbyterian Hospice a chance to expand and have space for staff meetings, patient and family meetings, and staff offices.

In 2007, PMMA extended hospice services to patients in their homes and other long-term care communities in the Farmington, Missouri, area.

Today, Heart & Soul Hospice continues its proud history of providing compassionate end-of-life care guided by Christian values.

In Missouri, we accept hospice admissions in Park Hills, Flat River, Bismark, Arcadia Valley, Desloge, Potosi, Bonne Terre, Ironton, Fredericktown and Farmington.
